每個人都會變老，關鍵是要怎麼變老。隨著台灣高齡與慢性病人口快速增加，如何讓病人獲得連續、整合的照護，已成為醫療體系的重要課題。「大家醫計畫」以家庭醫師制度為核心，協助病人整合慢性病用藥與生活管理，讓醫療從醫院延伸到日常，落實全人照護。

整合照護更便利 家庭醫師成為健康守門人
29年次、現年84歲的劉老先生，是計畫的參與者之一。他長年面臨血糖、血脂與高血壓等問題，但透過「大家醫計畫」，不必往返多家醫院，只需固定到家庭醫師陳宏麟的診所，就能整合用藥、安排檢查與追蹤指數，讓治療過程清楚、方便又省時。
陳宏麟醫師長期推動「醫養合一」理念，讓慢性病管理從醫療延伸至生活。他的小學導師正是劉老先生，如今角色互換。劉老先生笑說：「以前是我照顧他，現在是他照顧我，而且還照顧得更久。」

醫病關係進化 從治療疾病到陪伴生活
在持續追蹤下，劉老先生的血壓與血糖維持穩定，也養成了更健康的生活。「知道吃西瓜血糖會升高，就只吃一片；月餅最多也只吃半顆。」診所定期舉辦茶道、瑜珈等課程，協助病人建立規律作息。80多歲的他，精神與體力不輸6、70歲朋友；旅居國外的兒子看到父親能就近就醫、生活穩定，也感到安心。
「大家醫計畫」讓家庭醫師成為病人的主要照護窗口，整合不同科別資訊，減少重複用藥，並協助友善轉介心臟、腎臟等專科檢查。家庭醫師熟悉病史與生活狀況，能即時調整治療，使病人獲得安全而連貫的照護。

導入生活型態評估量表 讓照護更貼近日常
計畫導入的「生活型態評估量表」也是重要里程碑，記錄病人的飲食、喜好與睡眠狀況，協助醫師提供更個人化的建議。透過系統化紀錄，不僅病人更了解自身狀況，醫師也能全面掌握，讓照護更貼近生活。
這項制度不僅提升醫療效率，也讓醫師與病人關係更緊密。當民眾能在熟悉的社區就醫，醫療更便利，也更具信任感。
「大家醫計畫」持續在全台推動，透過家庭醫師的長期陪伴與跨院整合，讓慢性病患者少跑冤枉路、用藥更精準，讓醫療真正走入生活。對許多像劉老先生一樣的長者而言，這不只是就醫方式的改變，更是邁向健康老化的重要一步。
