10年糖友 受保腎護心卓越機構影響 燃起戒菸決心
今年60歲的張伯伯，已經患有糖尿病長達10年，過去他習慣在社區的家醫診所就診。三年前，他兒子在新聞中得知「保腎護心卓越機構」糖尿病診所的資訊，發現這些診所會專注糖友的心腎狀況做風險評估及衛教，而且剛好住家附近有獲得機構認證的診所，因此推薦父親前往看診。實際前往後，他們發現保腎護心卓越機構與一般診所不同，張伯伯分享說，過去在家醫診所就診時，醫師雖然也會針對他的心腎狀況做檢查，但並未花費太多時間進行詳細的解釋。相比之下，在卓越機構，醫師及衛教師會根據每個患者個別狀況進行風險評估，並且很有耐心的衛教相關資訊，讓病人可以了解每個資訊背後的的重要影響。張伯伯說，以前醫師常叫我要戒菸，我都聽聽而已，但聽了衛教師的說明後才知道抽菸對我有多危險，雖然我還沒完全戒掉，但跟過去相比已經少抽很多了，張伯伯笑著和我們分享轉診後最大的改變。
心腎共病症狀不易發現 定期評估風險 才能及早預防
隆安診所的羅文一院長指出，糖尿病患者可能面臨的心腎共病風險包含心肌梗塞、心血管阻塞、心臟衰竭及腎衰竭等併發症。心臟衰竭的常見症狀有走路時會喘、下肢水腫及容易疲倦，而腎衰竭通常沒有明顯症狀，需要透過抽血檢查才能確診。嚴重的腎衰竭可能會導致糖尿病患者需要接受洗腎治療。為了預防這些併發症發生，建議糖尿病患者應定期回診追蹤檢查。

羅文一院長提醒，糖友罹患心腎共病的風險因子包含抽菸、肥胖、三高(高血壓、高血脂、高血糖)、家族成員有心血管疾病或腎臟病病史、長期使用消炎藥及重口味的飲食習慣，這些都可能是引發共病的未爆彈。糖友來到保腎護心卓越機構，除了基本的血糖檢查外，醫師及衛教師會針對每個病人狀況進行評估及衛教，提供更全面的指導，幫助病人更積極控管心腎疾病的風險。

根據2023年國健署統計，全台約有200多萬名糖尿病友，每年近萬人因其死亡。由於糖尿病容易併發心臟衰竭、慢性腎臟病等心腎共病，嚴重影響患者的生活品質並縮短壽命，因此，糖尿病是需要高度關注的慢性病。
自2019起，台灣基層糖尿病協會與基層診所攜手推動「保腎護心卓越機構」認證，致力於協助糖友在每次的回診中有效控制血糖，更透過醫師及衛教師的指導，評估和檢測心臟及腎臟共病風險，提供全面的衛教宣導，讓糖友能更了解自身健康狀況，即時預防疾病的發生。目前民眾可透過「糖尿病保腎護心卓越機構」的衛教網站，查詢附近的認證機構，享受安心又便利的醫療服務。
