40 歲的李先生，術前身高173公分、體重180公斤，BMI超過60，屬於超級病態性肥胖，且合併嚴重睡眠呼吸中止症，一小時呼吸中斷次數高達80次，經過新光醫院一般外科主治醫師，同時擔任代謝管理暨減重手術中心主任方躍霖醫師的完整評估後，在今年4月，接受單吻合口雙通道縮胃迴腸繞道手術，三個月內已成功減重35.5公斤，體重仍持續下降，也逐步恢復身體健康狀態。
方醫師指出，減重病人在手術後的前三天，特別是BMI超過50的病人，是呼吸異常風險最高的「黃金觀察危險期」，減重手術的成功與否，不僅僅是要達成體重減輕，更是要同時維護術前術中術後身體健康及生命安全。方醫師也特別提醒，除了減重手術病人之外，其他接受一般外科或大直腸科之腹部手術的病人，也都必須留意術後的呼吸監測，因為呼吸異常是最容易被忽視，卻最危及生命的術後併發症之一，對病人與醫護團隊都至關重要。
高BMI減重病人 合併睡眠呼吸中止症術後風險高
為何減重病人在手術後的前三天，是呼吸異常風險最高的「黃金觀察危險期」？方醫師說明，肥胖病人本身就心肺功能較差，加上術後傷口疼痛導致不敢深呼吸與咳痰，因此更容易造成肺部換氣不足，使血氧濃度下降，若術前已合併睡眠呼吸中止症，情況會更嚴重。
尤其BMI超過50的病人，幾乎百分之百合併睡眠呼吸中止症，術後72小時內，常因呼吸道不穩定而出現缺氧、二氧化碳蓄積，甚至呼吸衰竭等危險。若此時忽視監測，恐進而導致心律不整、心臟衰竭，甚至危及生命。因此，他提醒，對高BMI的減重病人而言，術後「前三天」就是呼吸監測的關鍵期，務必要嚴格觀察。
呼吸監測 即時示警降低併發症發生
過去，高BMI減重病人術後多半需要進入加護病房接受呼吸監測。但方醫師說明，隨著AI輔助呼吸監測技術的導入，病人在一般病房即可接受持續監測，包括血氧、心跳、血壓與潮氣末二氧化碳濃度(EtCO2)等重要數據，並透過演算法整合為綜合指標，協助醫護團隊和病人家屬能及早發現異常。
當病人出現呼吸異常情況時，系統能即時示警，讓醫護人員與家屬迅速反應，避免錯過最佳處置時機。這樣的監測方式不僅可降低發生呼吸衰竭、心律不整、窒息等併發症而造成的生命風險，也能有效減輕護理人員的工作負擔，提升整體醫療安全與便利性。依照方醫師建議，減重案例李先生，在術後使用AI監測，順利度過術後三天的危險期，恢復良好。
一般外科和大直腸科之病人術後同樣存在呼吸風險
除了減重病人，其他腹部外科手術病人同樣存在呼吸風險。方醫師指出，腹部手術會影響橫膈膜活動，使肺部空間減少，導致病人呼吸功能下降。如果病人合併肥胖，風險會更高。研究與臨床經驗顯示，BMI超過35的病人，大多會有輕度到中度的睡眠呼吸中止症。
此外，即使BMI不算過高，但若出現「脖子較短、身體中心肥胖、四肢相對纖細」等體型特徵，也容易因為躺下時舌頭後倒造成呼吸道狹窄，而出現睡眠呼吸中止問題。這些族群在術後若不進行呼吸監測，就可能在恢復期中出現危險的呼吸抑制或窒息狀況，無法即時知道而做出相對應的救治。
方醫師強調，手術的成功不只在於成果，更必須著重於術後安全的全面守護。高風險病人及其家屬都應提高警覺，透過完善的術後監測，才能真正守護生命健康。
