清理耳垢是自己挖，還是到診所找專業的醫師處理呢？年紀60多歲老翁近3個月以來時常出現耳鳴、耳悶的症狀，且右耳聽力下降，還因爲聽不清楚，與妻產生口角影響夫妻感情。經耳鼻喉科醫師看診後，醫師發現右耳裡面竟有長達近2.5公分的耳垢！醫師花了5秒鐘的時間整條拉出，也解決了老翁聽力上的問題與困擾，耳鼻喉專科的張益豪醫師在臉書粉專「張益豪/免疫學.兒童成長.三官四感醫師」PO出照片，讓網友感到相當驚訝，表示「第一次看到這麼長的耳垢！」，醫師也幽默表示，耳垢當天已被老翁帶回，他還提醒老翁「千萬不能拿去泡酒！」
張益豪醫師於文中說明，耳垢就是俗稱的耳屎，它是「耳道皮膚的分泌物、脫落體、外界髒汙的共同混合物」，耳垢的主要功能其實是「保護耳朵」，防止灰塵、細菌和其他微生物進入耳朵深處，同時也有助於保持耳道的適當的潤濕狀態。但是，對於某些人來說，過多過滿的耳垢可能會導致耳朵悶塞、聽力較差等現象。

張益豪醫師提醒，一般而言，耳垢不需要特別去清潔，在日常咀嚼、顳頜關節活動的過程中，會有肌肉牽拉的動作，這些動作會使耳垢自動掉出，故不需要頻繁清理，它會自行脫落再被耳道排出。
此外，自行挖耳朵也有風險，容易使外耳道受傷，導致傷口會有組織液體滲出，患者就會覺得更癢、更想去挖而造成惡性循環。而潮濕的環境也創造出讓真菌容易滋生的環境，張益豪醫師建議有以下情形的民眾，可以定期到耳鼻喉科門診清潔與治療，不要自行清理耳朵，其中第5種情形很常見。
8種人建議定期至耳鼻喉科門診清潔耳垢
1. 外耳道天生較狹窄或彎曲者
2. 天生耳屎較多、較油又常堵塞者
3. 曾經接受過耳朵手術而引起結構異常者
4. 洗澡或游泳時，水經常流入耳道內，以致耳垢容易潮溼沾黏在外耳道上，進而造成阻塞者。
5. 很享受於清耳朵，但想交給專業的人清時。
6. 長時間需要使用耳塞或耳機的人。長期使用容易導致耳屎堆積。
7. 因工作與生活環境容易接觸灰塵、污染物的人。
8. 已經出現耳朵不適者，例如耳悶、聽力差、耳鳴、耳朵痛等等。
張益豪醫師提醒，若是以上需要定期清理耳垢的民眾，建議定期檢查耳朵，以便及早發現和處理耳道問題。保持耳朵清潔和健康，有助於預防耳道感染和聽力受損。
