2.複雜性尿路感染
複雜性尿路感染表示有潛在的泌尿系統疾病；常見如先天性的輸尿管分叉、膀胱輸尿管尿液逆流、尿道狹窄，後天性的疾病如尿路結石、神經性膀胱。所謂的「複雜性」，意味著這些潛在疾病造成的尿路感染不易治療，必須先治療潛在病因，才有可能治癒尿路感染。
臨床上，對首次的尿路感染病患，除了「尿液常規檢查」外，一般多不做進一步檢查（但新生兒除外，因為先天性疾病較多；幼兒即使第一次尿路感染，也要積極檢查其潛在病因），除非久治不癒或一再復發，才需安排進一步的檢查。
